+ Can I meet in person with a Care Counselor?

Our Care Counselors are always available to meet with you and your loved ones to discuss your family’s options. In fact, we prefer to get to know you in person. We are also open for tours of our residences during the times listed below. If you would like to schedule a tour outside of these hours, we would be happy to find a time that works best for you.

We understand that every individual is unique, and we offer many options for transitions out of the hospital. We provide short-term rehab, both in home and on our campus, as well as outpatient services and in-home care. + My dad needs more help than what he’s currently receiving. How do I get him more help?

With a full range of care solutions, our members, patients and residents often move through the landscape of older adult care with us, from service to service, whether it be moving from independent living to assisted living or returning home after a period of subacute rehab. Our Care Counselors will work with you to understand your situation and provide guidance about next steps in your loved one’s care journey.

We understand that for older adults who choose to stay at home, it’s often difficult to stay connected to the community. RiverSpring Social Day Care offers a safe and stimulating environment for seniors to socialize, exercise, and receive alternative therapies. We offer managed long-term care at home for older adults wishing to stay in the place they love. If you’re looking for additional tailored guidance, our Care Management team provides concierge services and guidance for older adults, their families, and caregivers. + Do I need a Certified Home Health Agency or a Licensed Home Care Agency?

Depending on your family member’s unique situation and needs while living at home, our Certified Home Health Agency or our Licensed Home Care Agency is here to help. RiverSpring Certified Home Health Agency offers short-term care, helping older adults transition home from their hospital or rehabilitation stay. For older adults who require managed long-term nursing care at home, RiverSpring Licensed Home Care Agency provides services from live-in care to aides and companions. + My aunt has Alzheimer's. What kind of options do you have?

Many of our care solutions support older adults living with dementia both in-home and within our nursing home.
