Your dedicated and expertly-trained Care Counselor is your guide through our full range of care solutions.

At RiverSpring Health, we consider ourselves great listeners who are fully committed to understanding your unique situation and helping you and your loved ones navigate the landscape of older adult care. Every person and every situation is truly one-of-a-kind, and we strive to create plans of care that holistically and compassionately address your particular needs. We have a simple 4-step process to help make sure you get the right care for you.

Our Process

Care Counseling

Your dedicated care counselor starts with active listening and problem solving to understand your unique situation.

Care Planning

Next, we’ll tailor a plan that best addresses your needs. To help you learn more, we’ll schedule a tour or an in-person meeting.

Receiving Care

Once a plan of care is determined, we’ll conduct a personalized clinical assessment wherever is most convenient for you and your family.

Care Transitioning

As your needs change throughout your time with us, your care counselor will ensure your safe transition home or into another service that is best for your ongoing care.

I tell everyone I speak with that I understand what they may be going through because I’ve been there with my own family.
