Our Process

Care Counseling

Your dedicated care counselor starts with active listening and problem solving to understand your unique situation.


Care Planning

Next, we’ll tailor a plan that best addresses your needs. To help you learn more, we’ll schedule a tour or an in-person meeting.


Receiving Care

Once a plan of care is determined, we’ll conduct a personalized clinical assessment wherever is most convenient for you and your family.

Care Transitioning

As your needs change throughout your time with us, your care counselor will ensure your safe transition home or into another service that is best for your ongoing care.

We help clients create unique plans of care, including those looking for care management, those seeking care in their own homes, those who are looking for care within the community, those looking for residential options, and those who need short or long term care.

+ Can I meet in person with a Care Counselor?

Our Care Counselors are always available to meet with you and your loved ones to discuss your family’s options--in fact, we prefer to get to know you in person. We are also open for tours of our residences during the times listed below. If you would like to schedule a tour outside of these hours, we would be happy to find a time that works best for you.
